  1. The company’s name and address.
  2. The company’s DOT number.
  3. The company’s operating authority.
  4. The company’s insurance coverage.
  5. The company’s safety rating.
  6. The company’s driver qualifications.
  7. The company’s vehicle maintenance records.
  8. The company’s driver logs.
  9. The company’s drug and alcohol testing records.
  10. The company’s hazardous materials safety records.
